The VA Ann Arbor Healthcare System is seeking a Clinical Pharmacist, GS-660-12 for the Ambulatory Care Service.

Tour of Duty: Day (Monday-Friday)



Major Duties

Clinical Responsibilities include:1. Maintains a practice site for ambulatory care clinical pharmacy services in the General Medicine Clinic and Specialty clinic areas including, but not limited to:Anticoagulation clinic

Medication refill clinic (HBPC)

Geriatrics Clinic

Lipid Management

pain Clinic

Arthritis Clinic

Pulmonary Clinic

Primary Care Clinics

Primary Care Pain Management Clinic

Smoking Cessation

Neurology Clinic

2. Provides Professional staff and patient consultations as appropriate within the Ambulatory Care Clinic Area.3. Promotes the appropriate, sage and effective use of medication therapy.4. Develops, coordinates and participates in programs in the Ambulatory Care clinics designed to improve patient compliance with prescribed medication and medication delivery to patients. 5. Presents new medications information. QA monitor results and DUE monitor results to the appropriate committee(s) when required.6. Keeps abreast of advances and changes in Ambulatory and geriatric medicine by participating in programs that provide education and training.7. Provides medication information to healthcare, professionals and patients through direct and director response to specific inquiries. 8. Provides medication refills and/or renewals as defined in the CPSC scope of Practice. B. Educational:


Supervises trans Pharmacy Practice residents and Doctor of Pharmacy Candidates on clinical rotations in Ambulatory care.

Provides in-service programs for the educational development of the Ambulatory Care staff as appropriate.

Participates in the training of professional staff, including pharmacy, medicine, nursing,  fellows, residents and students as appropriate.

Acts as a resource consultant and advisor where needed, as appropriate.

Contributes to Pharmacy Service publications (Drug Bulletin, Pharmacy Newsletter) and other medical center publications, when appropriate.

Promotes education for patients as approved by the Preventive Medicine/Patient Education Committee and Professional Practice Bureau.

C. Research and Publications:


May assist in research efforts by other healthcare professionals when appropriate.

May actively participate in research activities as defined in the CPS Scope of Practice.

Provides professional and technical assistance to research investigators when needed.

May publish research and/or non-research related articles in peer-review journals, independently or as a contributing member.

D. Program Management:


Assists the Ann Arbor Outpatient Pharmacy Supervisor and the Toledo Outpatient Supervisory Pharmacist in coordinating staff pharmacist involvement in ambulatory care clinical pharmacy services.

Assists in directing DUE activities in the ambulatory care setting.

Assists in QA activities in the ambulatory setting.

Qualifications

a) Citizenship:  Citizen of the United States.



(b) Education:  Graduate of a degree program in pharmacy from an approved college or university.  The degree program must have been approved by the American Council on Pharmaceutical Education (ACPE), or prior to the establishment of ACPE, have been a member of the American Association of Colleges of Pharmacy (AACP).



(c) Licensure:  Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.



(d) Experience:  Must have one (1) year specialized experience equivalent to the GS-11 grade level

b) Education:  Graduate of a degree program in pharmacy from an approved college or university.  The degree program must have been approved by the American Council on Pharmaceutical Education (ACPE), or prior to the establishment of ACPE, have been a member of the American Association of Colleges of Pharmacy (AACP).


You must be a U.S. citizen to qualify for this position.

Will be subject to the provision of the VA's Drug-Free Federal Workplace Policy, which includes both pre-employment/placement and random drug testing for the use of illegal drugs throughout employments.  Applicants who refuse to be tested will be denied employment with the VA.



You must have a certification, licensure, or registration credentials in Licensure for this position. In some cases, these may be substituted for experience and education.
