Major Duties
Incumbent of this position performs technical work necessary to meet regulatory requirements and facility policies regarding the processing and maintenance of medical record data. Codes in-patient discharges and out-patient encounters in accordance with ICD-9-CM and CPT coding systems. Assures completion of the Patient Treatment File (PTF) for submission to the national database. Performs difficult coding in accordance with procedures as outlined in the International Classification of Diseases, Diagnostic, and Statistical Manual of Mental Disorders. Ensures all diagnoses and procedures that impact reimbursement are identified and documented by the physician and coded accurately. Responsible for correcting coding errors. Analyzes all medical records for completeness, accuracy, and compliance. Finds omissions and discrepancies and corrects as needed. Codes outpatient encounters in accordance with CPT-4 system. Enters data into the Patient Care Encounter (PCE) for submission to the national database. Provides on-the-job training for students and new employees. Provides support to the admission/discharge function and record room duties. Performs medical record reviews regularly.

Qualifications
BASIC REQUIREMENTS:
a. Citizenship: Must be a US Citizen;
b. Experience and Education:
(1) Experience - Must have two (2) years of experience that demonstrates the ability to perform the work, or provides familiarity with the work. Six months of the required 2 years MUST have provided the knowledge, skills, abilities and other characteristics (KSAs) needed to perform medical records technician work;
OR
(2) Education – Completion of an associate’s degree with a major field of study in Medical Record Technology/Health Information Technology which was accredited by the American Health Information Management Association (AHIMA) at the time the program was completed.
OR
(3) Equivalent combinations of experience and education may be qualifying in some cases.
c. English Language Proficiency: Must be proficient in spoken and written English.
In addition to meeting the basic requirements, applicants MUST have 1 year of medical record technology experience equivalent to at least the GS-06 grade level. This includes: a) knowledge of classification systems, such as current versions of International Classification of Diseases (ICD), Current Procedural Technology (CPT), the Systematized Nomenclature of Medicine (SNOMED) or Healthcare Common Procedure Coding System (HCPCS), and skill in applying coding knowledge to a wide range of specialty areas; b) knowledge of Joint Commission on Accreditation of Healthcare Organizations (JCAHO) as well as Medicare/Centers for Medicare & Medicaid Services (CMS) and/or
VHA medical record documentation requirements; c) ability to communicate with clinical staff on coding and documentation issues such as recording inpatient and outpatient diagnoses and procedures, the use of encounter forms, the correct sequencing of diagnoses/procedures, and/or the relationship between health care documentation and code assignment; d) ability to research and solve complex questions related to coding and documentation, such as adequate health information documentation, coding, billing, resident supervision, etc.; and e) skill in reviewing and correcting system or processing errors, reviewing transmission reports, and ensuring all assigned episodes are complete and accurate as appropriate to the duties assigned. Certified coder preferred.
Applicants must meet the grade requirements as stated in VA Handbook 5005, Part II. Grade determination is based on the qualifications standards and determined by the Medical Records Technician Professional Standards Board.
VERIFICATION OF EDUCATION: Education must be accredited by the American Health Information Management Association (AHIMA) at the time the program was completed. Therefore, applicants must report only attendance and/or degrees from schools accredited by accrediting institutions recognized by the U.S. Department of Education.
