Job Listing: Care Manager - Telephonic

The Care Manager assesses the patient's current health status and understanding of their disease processes and access to care, identifies gaps or barriers that would prevent the patient from the following through with a recommended treatment plan consistent with optimal care for complex cases, and identifies and implements interventions and resources that address an close gaps in care, while continuing to assess the patient's current health status and knowledge and ability to manage their condition. This position is telephonic.
