We are looking for:
Cardiac Nurse Program Coordinator
- Assessment of status of current cardiac program with gap analysis and action steps for improvement (business plan review)
- Assessment of non-primary angioplasty program
- Site visits of similar programs to network and plan program enhancements
- Planning, developing and implementing the elective angioplasty program (policies, procedures and SOC) – CPORT II
- Quality management of cardiac program against evidence based cardiac practices and nursing standards of care
- Mentoring, precepting and developing staff against developed standards/goals/and outcomes
- Responsible for management/maintenance and reporting of CPORT I data and collaboratively managing CPORT II data with research nurse
- Analysis and monitoring of data to achieve targets in accordance with research protocol
- Navigator for patients through the “cardiac pathway” from pre admission through discharge and follow up.
- Ambassador for the cardiac program to physicians, IDT members, patients, families and others who are a part of services
- Facilitates and participates in departmental conferences (monthly M/M conference /others)
- Review patient satisfaction data and coordinates with Director regarding specific action planning for improvements
Qualities:
¿ Minimum BSN
¿ Strong cardiology background (medical and surgical) – experience in cath lab with invasive cardiology procedures – (cath, PCI. Devices)
¿ Past independent roles – case management, coordinator, research background a plus
¿ Visionary thinker – experience with program development/assessment/changes
¿ Good written skills
¿ Ability to develop and mentor staff
¿ Excellent communicator – especially with physicians and office staff
¿ Quality focused
¿ Critical thinker and excellent problem solver
¿ Able to mobilize a diverse team and establish accountability/desired performance standards and clear outcomes
¿ Able to multi-task and work with in collaboration with Director/Physician Leader/IDT members
