Job Listing: Case Manager

One of Aerotek Medical's most successful clients is looking for a case manager to join their growing team.

This positions will work as the liaison between the HMO and physicians performing care. The main function is to manage patients through the process.

The Case Manager performs utilization and case management activities onsite and/or telephonically including inpatient pre certification, concurrent and retrospective review; as well as discharge planning. The Case Manager provides for the complete, accurate and timely processing of utilization and case management data necessary to provide analysis, trending.

Collects appropriate clinical information to make a determination of medical necessity for reviewed service. Begins the discharge planning process upon admission or pre certification and completes a social assessment on initial review. Coordinates care with the members HMO as required.

Minimum Qualifications:
-RN or LPN (current IL license).
-3 yr experience in utilization review, quality assurance, discharge planning, or other cost management program required, preferably in managed care.
-3 yr clinical experience (strong medical/surgical preferred)
