Job Listing: Chart Review Nurse

General Responsibilities
- Accurately and efficiently perform medical record reviews
- Accurately identify ICD-9 Codes which capture HCC Diagnosis
- Navigate through Medical Chart Tracking System
- Properly distribute documents to applicable casework files
- Follow all HIPAA patient confidentiality requirements

Requirements
- Current Registered Nurse (RN) or Licensed Practical Nurse (LPN)
- Minimum of 3-5 years clinical experience
- Knowledge of HCC Codes
- Ability to manage and meet deadlines
- Detail oriented
- Effectively navigates telephony and technology
- Basic knowledge of Adobe Acrobat
- Proficiency in Microsoft Office Suite
- Must have excellent communication skills
- Demonstrates flexibility and ability to adjust to change
- Demonstrates follow-through on assignments
- Positive attitude, solution oriented
- Ability to receive and process performance feedback
